Our office will contact your insurance company to receive an estimate of coverage for the procedures being rendered. This estimate is based on the policy maximum and deductible. The difference between the estimated insurance reimbursement and procedure fee will be the responsibility of the patient/guarantor. This estimated patient share will be collected at the time of service. We are always able to send a formal Predetermination to your insurance company prior to services which can take two to four weeks to process. All remaining balances after your insurance processes your claim is the responsibility of the patient/guarantor with payment expected within 15 days of your statement date.
