We accept Cash, Visa, and MasterCard. We offer a medical / dental credit card called Care Credit. For more information regarding Care Credit, please call our office or access the website directly at http://www.carecredit.com. Our office will also set up a pre-pay payment plan with the last payment due on the day of the procedure.

Notice of Privacy Practices for Protected Health Insurance

Thank you for choosing Dr. Jason Hehr and his staff to take care of all your dental and medical needs. Our patients are greatly appreciated and we want your visits to be pleasant and stress-free.

We will need to make a copy of a picture ID, dental and medical insurance cards at your first visit.

At your first visit, we will collect a consultation fee. However, we will gladly file your insurance for reimbursement and any overpayment will be applied to your surgery fee.

At the end of your consultation appointment, each patient will be given a proposed treatment plan. The proposed treatment plan includes the fee for surgery and the estimated insurance reimbursement.

Your insurance company will be contacted to verify eligibility and an estimated percentage of coverage. The difference between our fee and the estimated insurance percentage will be collected on the day of surgery.

The responsible party will be responsible for all balances due on the account if the insurance fails to pay within 60 days and any costs if the account is placed with a collection agency.
